Go to - ODJFSBenefits.Ohio.gov - and click on the "Apply for Benefits" section

Note:   You must have an active e-mail address to setup an account.

-OR- Click the link below next to the type of assistance you would like to apply for, fill out the form and then print it. YOU MUST SIGN the form before mailing it. After signing the form you can then scan & email the form to info@wcdjfs.org.

You can also print the form and either fax (MUST BE SIGNED) it to us at (740) 373-9771, put it in the drop-box outside our building or mail it to:

One of our caseworkers will process your form and get back with you as soon as possible.

Note:   We need to have a phone number on the application so a casemanager can contact you for the interview. Anyone that does not have a phone will have to drop off the application and an appointment will be scheduled for you to come back in for the interview.
